PLEASE NOTE: WE WANT THE PERSON FOR WHOM OUR PRAYERS ARE BEING REQUESTED TO BE THE PERSON WHO FILLS OUT THIS FORM - UNLESS DUE TO THEIR ILLNESS THEY ARE INCAPABLE OF DOING THIS FOR THEMSELVES, THEN IT CAN BE DONE BY A CLOSE RELATIVE. We feel that by being an active participant in the reqest, it makes our connection with the person seeking the healing stronger and more direct.
